Case Study 01
Specialist healthcare practice · Backend Glow-Up — current paid client
This specialist practice already had many of the things businesses are usually told they need in order to scale:
The problem was not that the practice was disorganized.
The problem was that too much of the operation still depended on a few experienced people — especially the Founder/Physician — knowing how to connect everything.
The Founder described it early in the engagement:
“Everything kind of stays in my head or on my calendar.”
Patient information existed.
Processes existed.
The team knew what they were doing.
But an event happening in the business did not always automatically become the right next action, for the right person, with the right level of urgency.
That meant the Founder was still being pulled into routine questions and coordination.
Her description was simple:
“I have to answer every question.”
01
We started by studying how the practice actually operated — not just how its existing processes said it should operate.
We followed real patient journeys across areas such as:
The deeper issue was not one broken workflow.
It was that the business still relied heavily on people to translate information into action.
The EHR remained important.
But it was not designed to answer every operating question the team needed answered during the day.
02
This could not be solved by giving the practice a rigid checklist.
Healthcare is highly variable.
Different patients need different:
And some decisions should never be automated.
So the work became about separating:
what the system can carry
from
what must remain human judgment.
That distinction became foundational.
The goal was not to make every patient follow exactly the same process.
It was to create a consistent operating structure capable of supporting highly variable work.
03
During Diagnose and Architect, we redesigned how the practice should handle:
Different team members no longer need the same view of the business.
Administrative staff need administrative work.
Treatment staff need treatment information.
The Founder/Physician needs the decisions, escalations, and clinical issues that genuinely require her judgment.
The operating model is being designed so the system carries more of the coordination while the people retain the expertise.
04
One of the most important changes was separating a decision from the work created by that decision.
For example, a clinical decision might be complete.
But that decision may create:
Previously, that follow-through could still depend on somebody remembering it.
The new model is designed so those next steps remain visible and owned until they are actually complete.
05
Before full development, we translated the operating model into a working prototype and tested it against real scenarios.
The client responded:
“You’re capturing way more than I would have even anticipated.”
Later:
“This is perfect.”
“You’re getting me excited.”
And during the connected-system review:
“I’m actually blown away how well you’ve captured everything.”
The purpose of that stage was not to impress the client with a dashboard.
It was to confirm that the future operating model reflected how the practice actually needed to work.
06
Diagnose and Architect are complete and approved.
Build is actively underway.
The client has now logged into the working application, created test patients, and begun interacting with the system directly.
After testing the early Build, the client said:
“I don’t feel like anything felt confusing.”
And described it as:
“So simple, but effective.”
Most importantly:
“Based off of everything that I saw here, it was exactly what was in my brain.”
The Build is now being deepened with additional workflows, management visibility, and carefully bounded AI support.
07
The business worked because experienced people carried much of the operating logic themselves.
The designed operating system carries more of the:
The people retain:
The Founder described the bigger opportunity this way:
“If I can get this systemized, it can free up our mind to think so big.”
Current engagement. Build is active. Final post-implementation outcomes are not yet being claimed. Identifying client, staff, and patient information has been anonymized.
Your business does not have to look like this business for the operating problem to be similar.
If growth still depends on you remembering, checking, connecting and translating what happens next, the Backend Glow-Up is designed to change that.
